The 2nd card is a No Statement Card. If you are in a self-defense incident or anytime you are in a situation where police are involved, if you don't want to make a statement, just hand this card to the officer. It tells the officer you do not want to make any statement because you have been advised to always speak to a lawyer so you don't give up any of your Rights, however; the reverse side has information about you so the officer can start the incident report.
